Attachment 2 Application for Section 214 Authority Response to item 14: East Kentucky Network, LLC (“Applicant”) is a limited liability company organized under the laws of Kentucky. Five entities own equal 20% membership interests in Applicant: (1) Gearheart Communications, Inc. (“GCI”) owns 20% of Applicant. GCI is a provider of local exchange telecommunications services through the “d/b/a” name Coalfields Telephone Company. GCI is a Kentucky corporation that is owned by numerous shareholders, none of whom owns or votes a majority interest in GCI, although there are family relationships among various stockholders and Paul R. Gearheart has a controlling interest in GCI as that term is defined by FCC rules. His son, Paul D. Gearheart, participates on GCI’s behalf as a member of the Board of Applicant. Paul D. Gearheart is a United States citizen, and his address at GCI is PO Box 160, US Hwy 23, Harold, KY 41635. (2) Cellular Services LLC (“CSLLC”) (formerly named Cellular Services, Inc.) owns 20% of Applicant. CSLLC is organized under the laws of Kentucky and is a wholly-owned subsidiary of: Foothills Rural Telephone Cooperative Corporation, Inc. P.O. Box 240, 1621 KY Rte. 40W Staffordsville, Kentucky 41256 Foothills Rural Telephone Cooperative Corporation, Inc. (“Foothills”) is a corporation organized under the laws of Kentucky, and is a cooperative owned by its subscriber members. No subscriber owner of Foothills owns or votes as much as a 10% interest, or even a 1% interest, in Foothills. The General Manager of Foothills is Ruth Conley who is a United States citizen. (3) Mountain Telecommunications, Inc. (“MTI”) owns 20% of Applicant. MTI is a wholly-owned subsidiary of: Mountain Telephone Cooperative Corporation P.O. Box 399, 405 S. Main St. West Liberty, Kentucky 41472 Mountain Telephone Cooperative Corporation (“Mountain Telephone”) is a corporation organized under the laws of Kentucky, and is a cooperative owned by its subscriber members. No subscriber owner of Mountain

Telephone owns or votes as much as a 10% interest, or even a 1% interest, in Mountain Telephone. The General Manager of Mountain Telephone is Allen Gillum who is a United States citizen. (4) Peoples Rural Telephone Cooperative (“Peoples Rural”) owns 20% of Applicant. Peoples Rural is a provider of local exchange telecommunications services. Peoples Rural is a corporation organized under the laws of Kentucky, and is a cooperative owned by its subscriber members. No subscriber owner of Peoples Rural owns or votes as much as a 10% interest, or even a 1% interest, in Peoples Rural. The General Manager of Peoples Rural is Keith Gabbard who is a United States citizen. (5) Thacker-Grigsby Telephone Co., Inc. (“TGT”) owns 20% of Applicant. TGT is a provider of local exchange telecommunications services. TGT is owned by shareholders who, individually, own and vote less than a 50% stock interest in the company. As a result, no shareholder of TGT owns an effective 10% or greater interest in Applicant. Robert Thacker and William Grigsby share de facto control of TGT. Each is a United States citizen. Gerald Robinette serves as Chief Executive Officer of Applicant. Mr. Robinette is a United States citizen. 2
