STATEMENT OF PROFESSIONAL INSTALLATION FOR
CISCO WIRELESS ACCESS POINTS
In accordance with the FCC grant for the Cisco Systems, Inc. products listed in Table 1, these
devices can only be sold customers who utilize installers familiar with radio frequency
technology and the FCC regulations regarding transmitter output power, antenna gain, and
EIRP.

The purpose of this form is for the customer to confirm adherence to this requirement.
