1. Your application will be coordinated with our bureaus to be reviewed and voted on. In order for them to
run their analysis they need to know exactly where this experiment will be conducted and what the radius
of operation is. So please provide the radius of operation (in kilometer) from center point of the provided
coordinates.

2. There is restriction for the number of equipment and any number greater than 50 devices need a
justification. Please submit a statement explaining the reason for utilizing 150 devices vs. 50 devices and
answer to the following questions:

a) Are you requesting to perform a limited market study?

b) If a limited market study is going to be performed, will any fees be charged or will there be any
contractual arrangements between Intel Corporation, and the participants in this experiment?

c) Will all of the experimental devices be retrieved by Intel Corporation, from the participants upon
completion of this experiment?


The items indicated above must be submitted before processing can continue on the above referenced
application. Failure to provide the requested information within 30 days of March 06, 2009 may result in
application dismissal pursuant to Section 5.67 and forfeiture of the filing fee pursuant to Section 1.1108.
