STA # 0409-EX-ST-2004 was approved with specific conditions:

Note that in the TLS Acceptance Letter dated 10 March 2004 (attached), the FAA placed
specific limitations on the operations of TLS systems. In particular, offset approaches that had
been allowed under the previous STA for the TLS system at Hailey is no longer an acceptable
mode of operations for TLS systems. Therefore, this STA must be limited to straight in
approaches.

Adding the statement regarding "the offset approach procedure" does not appear to be in
conformance with the FAA restriction.

You are advised to discuss this with the FAA for further guidance in this matter. An updated
FAA letter may be all that is required to issue an STA with offset approach added. If you
discover that the FAA will not permit the evaluation of the offset approach procedure, please
respond by requesting that 0455-EX-ST-2004 be canceled.




The items indicated above must be submitted before processing can continue on the above
referenced application. Failure to provide the requested information within 30 days of
July 27, 2004 may result in application dismissal pursuant to Section 5.67 and forfeiture of the
filing fee pursuant to Section 1.1108.
