Dortch, Secretary Satelite Diision DearSecretary Dortch, IntornationaiBureau I am wriing to request cancellation of a satelite earth station, Call Sign E880961/File 001935—DSE—P/L—88, which is held by the Eisenhower Medical Center. The Eisenhower Medical Center has contracted with us to cancel this authorization, as they are dismanting their ground station. We were under the assumption that a Form 312 would suffice for this request. However, when 1 spoke with Towanda Bryant with the FCC International Bureau today, I was informed that I simply needed to write a formal letter to you requesting this.. 1 was also told there were no fees associated with this Aftached is the original signed copy of a Form 312, which Hospital Fa s Manager Fred Allard signed authorizing this cancellation. (We had him complete this before we knew better.) While not required, I felt you would prefer to have his signed release. 1 have also included a copy of the affectedlicense. Upon completion, I am requesting some written documentation that this has been done. You may either mail, fax, or e—mail this confirmation in whatever format is convenient for you.
