Attached to that filing was a letter from The State of Alaska Department of Commerce, Community and Economic Development, Corporations, Business and Professional Licensing, demonstrating that it had issued a Certificate of Amendment changing the name of Alaska Telecom, Inc. to Futaris, Inc. The purpose of this supplementary filing is to change the mailing address of Futaris, Inc. to read as follows: 301 Calista Court, Suite B Anchorage, Alaska 99518 This change is reflected in the "filer information" data entered on the IBFS form. Note that the FRN registration numbers associated with Futaris, Inc. have already been updated to show this address change.
