In section 5 of the Application, the “Name” of the Section 214 Authorization Holder was inadvertently listed as Mr. Roger Bundridge, but the “Name” should have been listed as Northwest Missouri Cellular Limited Partnership, and Mr. Roger Bundridge should have been listed in the “Attention” field of this section. The remaining information in section 5 is otherwise accurate and correct. In section 6 of the Application, the “Name” of the Transferor was inadvertently listed as Mr. Roger Bundridge, but the “Name” should have been listed as Northwest Missouri Cellular Limited Partnership, and Mr. Roger Bundridge should have been listed in the “Attention” field of this section. Also, the DBA Name field should have been left blank. The remaining information in section 6 is otherwise accurate and correct.
