Attachment 1 Answers to Questions 10, 11 and 13: Authority is hereby requested for a transfer of control of Sierra Telephone Long Distance (“STLD”), from Harry H. Baker, Jr. to John H. Baker. The transfer of control occurred on January 12, 2015 when John H. Baker became successor and sole trustee for each of the two trusts that hold a combined 98% of the voting stock of Sierra Tel Communications Group, the sole stockholder of STLD. The attached “Joint Application” of Sierra Telephone Company, Inc. and STLD includes complete responses to Questions 10, 11 and 13. In a separate application filed with the International Bureau the parties are requesting Special Temporary Authority for the continuing operations of STLD while this application for transfer of control authorization is pending.
