Supplement to the Application of Slappey Telephone, Inc. and Slappey Communications, LLC ITC-ASG-20190612-00120 The following is a supplement to the Joint Application for Consent to Transfer Domestic and International Section 214 Authority from Slappey Telephone, Inc., an Alabama corporation, as transferor (“Slappey Telephone”), to Slappey Communications, LLC, a Texas limited liability company, as transferee (“Slappey Communications”). The following is provided in response to a request for additional information: 1. Slappey Telephone provides facilities-based, cloud based, and VoIP telecommunication services in the following four geographic areas in the state of Alabama: (a) Birmingham County (b) Montgomery County (c) Huntsville County (d) Mobile County 2. Slappey Communications does not currently provide international services and does not hold a section 214 international authorization. Slappey Communications intends to offer international services to its customers upon the approval of the assignment of the required licenses from Slappey Telephone to Slappey Communications. 3. Slappey Communications and Slappey Telephone intend for Slappey Telephone to assign its 214 authorization to Slappey Communications. 4. No other person or entity owns a 10% or greater interest in Baymark Management, LLC. (c) Zarkava Holdings, LLC, a Texas limited liability company is 100% owned by Adnan Rehmatullah, a citizen of the United States of America. The address for Zarkava Holdings, LLC is 2021 Lambor Lane, Lewisville, Texas 75056. No other person or entity owns a 10% or greater interest in Zarkava Holdings, LLC. (e) The following persons or entities own a 10% or greater interest in Novus Method, LLC, an Alabama limited liability company: Wesley J. Slappey, a citizen of the United States of America and William J. Slappey III, a citizen of the United States of America. No other person or entity owns a 10% or greater interest in Novus Method, LLC. (f) The following persons or entities own a 10% or greater interest in Argonian, LLC, an Alabama limited liability company: William J. Slappey IV, a citizen of the United States of America and Sarah Elizabeth Leona Slappey, a citizen of the United States of America. No other person or entity owns a 10% or greater interest in Argonian, LLC. 6. Please see responses in 5 above. 7. Please see responses in 5 above. 8. Effective May 30, 2019, Slappey Communications purchased from Slappey Telephone substantially all of the assets of Slappey Telephone. In order to ensure an orderly transition of Slappey Telephone’s licenses, all licenses held by Telephone were excluded from the transaction. Slappey Telephone is still providing the applicable services until the Commission’s approval of the transition of the licenses.
