Answer to Question 11 There are no indirect owners of applicant which own 10% or more of the equity of the Applicant. Applicant is 100% owned by Lakeland Communications, Inc. (LCI), a domestically organized (Wisconsin) holding company -- there are no direct or indirect owners of LCI which own 10% or more of the equity of LCI. Answer to Question 13 Lakeland Communications, Inc. (LCI) is a domestically organized (Wisconsin) holding company which was, until recently, the 100% owner of the following domestically organized (Wisconsin) companies: Lakeland Telecom, Inc. (FRN 0003744554), Milltown Mutual Telephone Company (FRN 0003744430), and Luck Telephone Company (FRN 0003744398). As part of a pro forma corporate reorganization, LCI merged Lakeland Telecom, Inc. and Milltown Mutual telephone Company into Luck Telephone Company (surviving entity). To complete the reorganization, the name of the surviving entity was changed to Lakeland Communications Group, LLC dba Lakeland

Communications. Pertinent to the instant transaction: Lakeland Telecom, Inc. is the holder of the following Global or Limited Global Resale Service Authorization: File No. ITC2142000071700412. The instant notification is being filed to inform the Commission of the corporate reorganization and to change the name on the Global or Limited Global Resale Service Authorization from Lakeland Telecom, Inc. to Lakeland Communications Group, LLC dba Lakeland Communications.
