Chandler Blvd. Suite 111​216 Phoenix, Arizona 85048. No other individual or entity holds directly or indirectly a 10 percent or greater ownership interest in FTI. There are no interlocking directorates -- no individual is an officer or director of FTI as well as an officer or director of an entity that is a foreign carrier​. Answer to Question 15 Section 63.18(d): FTI certifies it has not previously received international Section 214 authority. Section 63.18(e)(3): Not applicable. Section 63.18(g): FTI will use previously authorized facilities to provide its facilities-based services and thus this Application is categorically excluded from environmental assessment pursuant to Section 1.1306 of the FCC Rules
