ATTACHMENT 1 Before the FEDERAL COMMUNICATIONS COMMISSION Washington, D.C. 20554 In the Matter of: ) Maquoketa Valley Rural Electric ) Cooperative d/b/a Maquoketa Valley ) Electric Cooperative Application Under Section 214 ) As Amended, for Global Authority For the Provision ) File No. _____________________ Of Resold International ) Switched Services Between the U.S. and Various ) International Points ) Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative attests to its qualification for streamlined processing under 63.12 as follows: a) Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative is not a foreign carrier, nor does it control a foreign carrier as defined in Section 63.09(d), in any destination market. b) No two or more foreign carriers, or parties that control foreign carriers, own more that 25% of Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative or are parties to or beneficiaries of a contractual relation affecting the provision or marketing of international basic telecommunications services in the United States. c) No owner of Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative controls a foreign carrier in any country. d) Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative has no affiliation with a dominant U.S. carrier whose international switched or private line services Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative seeks authority to resell. e) Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative is not requesting authority to provide switched services over private lines to countries not previously authorized for service by the Commission. Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative responds to 63.18 (d) and (e) as follows: (d) Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative has not received previous authority under Section 214. (e) Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative requests Section 214 Authority to operate as a resale carrier pursuant to the terms and conditions of Section 63.18(e)(2). Maquoketa Valley Rural Electric Cooperative d/b/a Maquoketa Valley Electric Cooperative will comply with the terms and conditions contained in Section 63.21 and 63.23 of the Commission's Rules. 47 C.F.R. 63.21 and 63.23.
