(“LPC LD” or “Applicant”) are as follows. LPC LD is wholly owned by La Porte City Telephone, Inc. (“LPC Telephone”), a publicly traded Iowa corporation. LPC Telephone has only one shareholder (Susan Robertson) with a 10% or greater interest. Susan Robertson, a U.S. citizen and officer and director of both LPC LD and LPC Telephone, holds an 11% ownership interest in LPC Telephone. LPC LD and LPC Telephone have a common Board set forth below. All board members are U.S. citizens. There are no interlocking directorates with application foreign carrier. LPC Long Distance, Inc.
