Attachment 2 Application for Section 214 Authority Response to item 14: Clear Stream Communications, LLC (“Applicant”) is a limited liability company formed under the laws of North Carolina. Slayton S. Stewart, a United States citizen, serves as Managing Manager of Applicant. Applicant has four member owners: 1. Carolina West Wireless, Inc. (“CWW”), a North Carolina corporation whose address is PO Box 959, 1307 Curtis Bridge Road, Wilkesboro, NC 28697, owns a 10% membership interest in Filer. Slayton S. Stewart, a United States citizen, is Chief Executive Officer of CWW. CWW has three stockholders: (a) Skyline Telephone Membership Corporation (“Skyline”), a North Carolina corporation whose address is PO Box 759, West Jefferson, NC 28694-0759, owns a 47% stock interest in CWW. Skyline is an incumbent local exchange carrier that is organized as a cooperative in which subscribers are the company owners. No subscriber/owner has as much as a 1% ownership or voting interest in Skyline. (b) Surry Telephone Membership Corporation (“Surry”), a North Carolina corporation whose address is PO Box 385, Dobson, NC 27017-0385, owns a 33.3% stock interest in CWW. Surry is an incumbent local exchange carrier that is organized as a cooperative in which subscribers are the company owners. No subscriber/owner has as much as a 1% ownership or voting interest in Surry. (c) Wilkes Telephone Membership Corporation (“Wilkes”), a North Carolina corporation whose address is 1400 River Street, Wilkesboro, NC 28697- 2108, owns a 19.7% stock interest in CWW. Wilkes is an incumbent local exchange carrier that is organized as a cooperative in which subscribers are the company owners. No subscriber/owner has as much as a 1% ownership or voting interest in Wilkes. 2. SkyBest Communications, Inc. (“SkyBest”), a North Carolina corporation whose address is PO Box 759, West Jefferson, NC 28694-0759, owns a 30% membership interest in Filer. SkyBest is a wholly owned subsidiary of Skyline. 3. Piedmont Communications Services, Inc. (“Piedmont”), a North Carolina corporation whose address is PO Box 385, Dobson, NC 27017-0385, owns a 30% membership interest in Filer. Piedmont is a wholly owned subsidiary of Surry.

4. Wilkes Communications, Inc. (“WCI”), a North Carolina corporation whose address is 1400 River Street, Wilkesboro, NC 28697-2108, owns a 30% membership interest in Filer. WCI is a wholly owned subsidiary of Wilkes. Response to item 15: With regard to Section 63.18(d) of the Commission’s rules, Applicant has not previously received authority under Section 214 of the Act. With reference to Section 63.18(e)(3), Applicant is not applying for authority to acquire facilities or to provide services not covered by paragraphs (e)(1) and (e)(2) of Section 63.18. Section 63.18(g) is also inapplicable because Applicant does not seek facilities-based authority under paragraph (e)(3) of Section 63.18. 2
