FRN 0021664876 Application for Authority to Provide Resale Service in Accordance with Section 63.18(e)(2) of the Commission’s rules, 47 C.F.R. § 63.18(e)(2) Question 14: Provide the name, address, citizenship and principal business of the applicant’s ten percent or greater direct and indirect shareholders or other equity holders, and identify any interlocking directorates. Response to Question 14: Cordova Long Distance, Inc. (“CLD”) is a wholly owned subsidiary of Cordova Telephone Cooperative, Inc. There are no other persons or entities with ten percent or greater direct or indirect equity interests in CLD. Cordova Telephone Cooperative is a member-owned cooperative formed under Alaska Statutes Title 10, Chapter 25 as is principally engaged in the business of providing local exchange telecommunications services in Cordova, Alaska. Its address is 611 Second Street, P.O. Box 459, Cordova, Alaska 99547. CLD has common directors with Cordova Telephone Cooperative, Inc., and with Cordova Wireless Communications, Inc., neither of which is a foreign carrier. Question 15: Respond to paragraphs (d) (e)(3), and (g) of Section 47 C.F.R. § 63.18. Response to Question 15: Section 47 C.F.R. § 63.18(d): To the best of CLD’s knowledge, CLD has not previously received authorization under Section 214 of the Act. Section 47 C.F.R. § 63.18(e)(3): CLD is not applying for authority to acquire facilities or to provide services not covered by paragraphs (e)(1) and (e)(2) 47 C.F.R. § 63.18. Section 47 C.F.R. § 63.18(g): CLD is not seeking facilities-based authority under 47 C.F.R. § 63.18(e)(3).
