A response to paragraphs (d), (e)(4) and (g) of Section 63.18; Section 63.18 paragraph (d), Applicant has not previously received authority under Section 214 of the Act. Section 63.18 paragraph (e)(1), Applicant is applying for authority to acquire facilities or to provide services not covered by paragraphs (e)(2) through (e)(4) and will comply with the terms and conditions contained in 63.21 & 63.22.
