The following information is provided with respect to shareholders of 10% or greater of the equity interests of Applicant: The only shareholder of 10% or more of the equity interest in Smithville Telephone Company, Inc. is the Patricia A. Earles Irrevocable Trust (the "Trust"), which owns approximately 91% of the equity interest in Applicant. The trustees of the Trust are: William H. Earles, Dwane Glancy and David McCrea. The principal business of Messrs. Earles and Glancy is the provision of telecommunications services. Their principal business address is 1600 W. Temperance Street, Ellettsville, IN 47429. Mr. McCrea is an attorney. His principal business address is P.O. Box 1310, Bloomington, IN 47402. All Shareholders, Trustees, Officers and Directors of the Applicant are U.S. Citizens. There are no interlocking directorates with a foreign carrier.
