                                    SUPPLEMENT

         Qwest Corporation (“Qwest”) hereby supplements its application for an
Experimental Radio Station License with additional information supporting its request.
As set forth in its original submission (see FCC File No. 0228-EX-PL-2005), Qwest is
requesting experimental authority to deploy and operate up to 250 WiMAX end-user
devices within a 10 kilometer radius of two fixed base stations. Specifically, Qwest plans
to deploy and test up to 200 end-user devices at various locations near a proposed base
station site at Mead, Colorado, and up to 50 end-user devices near a proposed base station
site at Littleton, Colorado.

        Qwest, an acknowledged leader in the field of broadband distribution, seeks to
test and evaluate the utility and viability of WiMAX technology as an alternative means
of providing broadband access. In order to do so, Qwest needs to approximate, as close
as possible, the operating conditions under which a WiMAX network would be
commercially deployed. Toward this end, Qwest seeks to deploy a sufficient number of
end-user devices to test and evaluate the impact on system performance and throughput
resulting from dense concentrations of customers in close proximity to one another, as
well as the suitability of various frequency reuse schemes designed to maximize system
capacity. Qwest believes that it has requested the minimum number of units necessary to
accomplish these objectives.

       At the end of the experimental trial, Qwest will suspend base station operations
and collect all end-user equipment. Moreover, Qwest will exercise appropriate care to
ensure that all end user equipment is returned at the end of its experimental trial.
