Dear Sir or Madam:

On behalf of Medtronic Incorporated, transmitted herewith is an application for an STA to permit the
operation of an ultra low power medical implant. The unit is a modified version of a unit certified
under Part 15 of the FCC Rules, FCC ID: PHZ-BRAVO100. The modification consist of changing
the PRF to transmit at intervals of once per second. The certified unit transmits at a rate of once
every 12 seconds.
